Northwest Health - Starke Launches New Transitional Care Program
Knox, IN – For patients about to be discharged from the hospital, it’s common to worry about whether they are ready to manage their recoveries on their own. The rehabilitation process after some health events is as critical as the treatment itself, often playing a role in long-term outcomes and quality of life.
Northwest Health is pleased to announce a new Transitional Care Program at the Northwest Health – Starke hospital aimed at helping patients with this important phase of recovery. Sometimes referred to as a “swing bed” program by Medicare, this service allows patients to extend their recovery and receive rehabilitation in a safe, skilled nursing environment – at Northwest Health - Starke - after completing an acute care, three-night qualifying hospitalization.
The Transitional Care Program, which requires a physician referral, is ideal for patients who are recovering from surgeries, strokes or other health conditions that require additional support.
“The Transitional Care Program is designed for patients who need more time to strengthen, heal and get back on their feet before they return to their daily lives,” said Mark Casanova, Chief Executive Officer of Northwest Health. “The program is a bridge between hospital and home, offering the extra care and support patients need to recover safely and confidently. Northwest Health – Starke is the ideal location for this new service given the hospital’s capacity and skill set of its staff.”
Services in the Transitional Care Program at Northwest Health – Starke include:
- A safe, comfortable hospital environment with private rooms
- Physicians and registered nurses on duty around the clock
- Administration of IV fluids and medications
- Rehabilitation services, including physical, occupational and speech therapies
- On-site lab, radiology and pharmacy
- Respiratory therapy
- Skin and wound care
- Individualized care plan, care coordination and discharge planning by case managers
